Propolis

Propolis is what bees seal the hive with, and it's the mouth and throat where it earns its keep.

Mouthwashes, sprays and lozenges put it straight onto the tissue. That's how nearly all the research has been done. What ends up in the jar depends on which trees the bees visited, so brands vary.

Propolis is one of the few things where the format decides whether it works, so here's the triage our naturopaths run before picking one off the shelf.

What's propolis actually for?

The evidence concentrates on the mouth. Propolis mouthwash reduced severe oral mucositis across five randomised trials in cancer patients, and propolis formulations reduced dental plaque across 45 studies. There's a smaller body of work on swallowed propolis and blood sugar markers. If you want it for your mouth, pick a format that stays in your mouth.

Why does the label warn about allergic reactions?

Because propolis is a documented allergen, and Australian law requires the warning on every non-topical product: if irritation or swelling of the mouth or throat occurs, stop using it. Topical products carry a separate skin irritation warning and tell you to test before use. If you react to bees, pollen or tree resins, this isn't for you.

Why does propolis vary so much between brands?

Because bees make it from whatever trees grow near the hive. Composition shifts with geography, which is why Brazilian green propolis and a European propolis aren't the same material chemically. If that matters to you, look for a standardised extract where the characterisation is stated rather than a generic raw propolis.

How much, and when

Reference values. There's no Nutrient Reference Value for propolis, because it's a hive product rather than a nutrient.

Typical supplemental range. Trial doses vary with the format. Oral mucositis trials used propolis mouthwash rather than a stated milligram dose. A standardised Brazilian green propolis extract was studied at 900 mg a day for 10 days in hospitalised patients. The glycaemic meta-analysis pooled 21 RCTs across varying doses rather than settling on one.

Upper level. None is set, and Schedule 1 records no daily dose cap for any propolis form. The one hard number in the Determination is a composition limit rather than a dose: lead must be no more than 0.001% of the medicine.

Worth knowing. Match the format to the job — local formats for mouth and throat, capsules for the systemic evidence. Your needs depend on your age, diet and health circumstances — talk to one of our naturopaths.

The evidence, and its limits

  • Propolis mouthwash lowered the incidence of severe oral mucositis across 5 RCTs in 209 cancer patients (OR 0.35). (PubMed 30022350)
  • Across 45 studies, propolis formulations reduced plaque index versus negative control (SMD -1.11). (PubMed record)
  • Across 21 RCTs, oral propolis lowered fasting glucose by 9.75 mg/dL and HbA1c by 0.46%. (PubMed record)
  • C-reactive protein was not reduced overall across 20 RCTs in 1,139 adults, only at durations of 10 weeks or more. (PubMed record)
  • Composition varies enormously with the trees local to the hive, so two jars from different countries differ chemically. (DOI record)

How it works

Propolis is the hive's antiseptic, and most of what it's been tested for is local rather than systemic. Put it in contact with the tissue you want it to act on — the mouth, the throat, the gums — and the resins do their work where they land. That's why mouthwashes and lozenges dominate the trial evidence and why the cleanest result in the whole register is for mouth ulceration during cancer treatment. There's also a smaller body of work on swallowed propolis and blood sugar markers, but the composition problem cuts across everything: two jars from different countries aren't the same material.

The Australian rules

Propolis appears in Schedule 1 of the Therapeutic Goods (Permissible Ingredients) Determination (No. 2) 2026 as item 4160, with propolis balsam (4161), dry extract (4162) and liquid extract (4163) carrying identical conditions. Two things attach to all of them. First, a composition limit: lead is a mandatory component of propolis and must be no more than 0.001% of the medicine — an unusual requirement that reflects what the raw material actually contains. Second, two mandatory warnings split by route: topical products carry the skin irritation warning, and everything that isn't topical carries the allergic reaction warning. There's no daily dose cap on any propolis form and no age restriction, which is worth contrasting with royal jelly (item 4373), a neighbouring hive product that carries "Not suitable for children". Permitted-ingredient status isn't a government endorsement of any product or claim.
