Methylcobalamin

What is methylcobalamin?

Methylcobalamin is vitamin B12 in its ready-to-use coenzyme form, the one your cells use directly. B12 supports healthy nerves, and this form comes as an easy sublingual tablet, spray or drop.

Is Methylcobalamin right for you?

Methylcobalamin is the form most people reach for in a sublingual or spray. These are the questions our team asks before recommending it.

A good fit if

  • You want a sublingual, spray or drop rather than a tablet to swallow
  • You're vegan or vegetarian and want the coenzyme form of B12
  • You've had gastric bypass surgery and need an oral option that matched injections in a trial
  • Your practitioner has suggested this form specifically, alongside your usual care

Check with us first if

  • You're on metformin long-term. It lowers B12 the longer you take it, and the risk rises with years of use.
  • You take a high-dose folate or folic acid supplement. Large folate doses can make your blood test look normal while nerve damage carries on.
  • You use nitrous oxide, medically or recreationally. It inactivates B12, and a normal blood test won't pick it up.
  • You take a proton pump inhibitor or another reflux medicine long-term.
  • You're pregnant or breastfeeding. Talk to your doctor before starting, especially in the first trimester.
  • You take any prescription medicine at all. Check with your doctor, pharmacist or our naturopaths first.

Common questions

Is methylcobalamin better than cyanocobalamin?

Not for raising your B12. The best comparison found no real difference between B12 forms or ways of taking it for that. Methylcobalamin does have the strongest trial evidence of the three oral forms for diabetic nerve symptoms, although those trials are mixed and mostly small.

Do I need methylcobalamin if I have the MTHFR gene variant?

We haven't found a trial that compares B12 forms by MTHFR result, so we can't say methylcobalamin works better for you than any other form on that basis. If you've had genetic testing, bring your results to a practitioner. They're better placed to advise you than a product label is.

Can I get methylcobalamin as an injection?

No, not legally as a listed medicine here. Australian rules limit this form to oral medicines only, such as sublingual tablets, sprays, drops and capsules, never an injection. Hydroxocobalamin is the form used for B12 injections in Australia, and that's always a decision for your doctor or practitioner.

How much, and when

Reference values. The NHMRC recommended daily intake for vitamin B12 is 2.4 µg a day for adults, rising to 2.6 µg in pregnancy and 2.8 µg while breastfeeding. Those figures are for the vitamin as a whole, not this form specifically. You'll find the full table by age on our vitamin B12 page.

Typical supplemental range. Australian sublinguals, sprays and drops commonly contain 500 to 1,000 µg of methylcobalamin, and some sublinguals go up to 5,000 µg. The trials used 1,000 µg a day, swallowed or taken under the tongue, in people who were low in B12, had trouble absorbing it or were managing diabetes.

Upper level. The NHMRC hasn't set an upper limit for vitamin B12 in any form. In its own words: "There are insufficient data to allow setting of a UL." Australian rules set no dose cap for mecobalamin either, only the condition that it's for oral use.

Worth knowing. What you need depends on your age, diet and health, so talk to one of our naturopaths.

Food sources

Food first, where the diet allows it.

  • Meat, fish and shellfish: the main dietary sources; the B12 in food is a mix of forms your body converts, including into methylcobalamin
  • Liver: the most concentrated source, though only about 11% of its B12 is actually absorbed
  • Eggs and trout: 24 to 40% of the B12 present is absorbed
  • Mutton and chicken: more than 60% of the B12 present is absorbed, the highest rate measured
  • Milk and dairy: about 30% of adult intake and 50% of children's intake comes from here

Before you start

If any of these sound like you, talk to one of our naturopaths or your doctor before you start.

  • You're sensitive to cobalt or vitamin B12. Allergic reactions, including anaphylaxis, have been reported with cobalamin treatment.
  • You've taken metformin long term. It lowers B12 the longer you take it, and the risk rises with years of use.
  • You take high-dose folate or folic acid as well. It can correct blood-count changes while nerve damage carries on unnoticed.
  • You use nitrous oxide, medically or recreationally. It knocks out B12, and a normal blood test won't pick it up.
  • You've used proton pump inhibitors or reflux medicine long term.
  • You're pregnant or breastfeeding. Talk to your doctor first, especially in the first trimester.
  • You have ongoing nerve symptoms like numbness, tingling, unsteadiness or memory changes. These need a diagnosis, not a supplement.
  • If you take prescription medicines, check with your doctor, pharmacist or our naturopaths before you start.

The evidence, and its limits

  • Methylcobalamin gives you vitamin B12; nerve problems linked to deficiency affect about 75-90% of people who are deficient, often before blood changes show up. (NHMRC nutrient reference values)
  • Methylcobalamin is one of B12's two active forms, and the body uses it directly in one of the vitamin's two enzymes.
  • In Australia it's called mecobalamin, and it's allowed in listed medicines taken by mouth only. (Schedule 1, item 3236)
  • In people with diabetes, a 12-month trial of 1,000 µg a day improved nerve function, pain and quality of life. (PubMed 33513879)
  • A review of 15 trials found no benefit for pain, and rated most of those trials at high risk of bias. (PubMed 32716261)
  • No trial we hold shows methylcobalamin raises B12 better than cyanocobalamin in someone who isn't deficient. (PubMed 38231320)

How it works

Vitamin B12 does two jobs in your body, and methylcobalamin is the ready-made form for one of them: turning homocysteine into methionine, a reaction that runs all the time inside your cells. If you take cyanocobalamin or hydroxocobalamin, your body first has to remove part of the molecule and rebuild it before that reaction can use it. Methylcobalamin skips that step for this job, because it's already the working form. It still needs converting for B12's other job, the one inside your mitochondria (your cells' power plants). In Australia this form is permitted only in oral medicines, which is why it's behind most sublingual tablets, sprays and drops and almost never an injection. If you run low on B12, your nerves can suffer before a blood test looks wrong.

The Australian rules

You won't find 'methylcobalamin' on Australia's permitted ingredients list (Schedule 1). It's listed under its international name, mecobalamin, item 3236, and it's allowed in listed medicines with one condition the other two oral forms don't have: "Only for use in oral medicines." That's why you'll see it in tablets that dissolve under the tongue, sprays, drops and capsules, but never in an injectable or skin product. No maximum daily dose or required warning is set for it. Being allowed means it can legally be used in a listed medicine; it isn't the government endorsing any product or claim.

Read the instrument: Schedule 1, item 3236.
